On receipt of the email invitation to complete the 'Application for Accreditation' the below evidence is required to be uploaded with your application form. It is advisable to prepare this documentation in advance to expedite your application, as some may require further steps to prepare.

You will require the following key documentation in PDF format to support and attach to your application including:

• Your Medical Indemnity Insurance
• A current Police Check
• A Working with Children Check
• Details of up to 4 Referees, 2 that have worked in your Scope of Clinical Practice (Names & Emails)
• Your recent CV
• Documentation supporting your Scope of Clinical Practice e.g. GESA Logbook
